The purpose of this survey is to investigate whether and how Zambia’s local government’s environmentally responsible behavior resolves or exacerbates social welfare issues arising from inappropriate mining practices and the harm to the ecological environment. Furthermore, the survey asks questions about the effectiveness or otherwise of government institutions and mining firms' value co-creation toward sustainable mining and environmental protection practices in line with resolving social welfare issues such as environmental pollution-related health diseases, poverty, food security, and social equity. Accordingly, the survey’s objective is to gather data on these factors/practices, solely for research investigation and formulation of insightful practical implications for both the Zambia government and its oversight responsibilities on mining firms in resource-endowed regions of the country. There are no wrong or right answers, therefore, respondents are encouraged to answer each question with full knowledge and with ultimate sincerity. All responses will be anonymous and will be treated with the highest confidentiality. Thank you for your cooperation.
